This website provides essential material for local health and social care systems effectively to mobilise the care home Capacity Tracker.
The award winning web-based Capacity Tracker was built in partnership with NHS England, Local Authorities and care home providers. Designed to minimise the many speculative calls each day to care homes trying to find a vacancy it enables care homes to instantly share their available capacity in real time 24-7 using any device connected to the internet, which in turn allows users such as discharge teams, social workers to quickly search for those care home vacancies across England based on affordability, location and service quality, plus much more.
Once a care home has been identified as the appropriate place of care for the individual, the Capacity Tracker helps the person and their family to make the right choice and ensure that they don’t stay in hospital any longer than is necessary. This helps to optimise patient flow through hospitals by minimising avoidable Long Length of Stay (LLoS) and Delayed Transfers of Care (DToCs) so it’s better for patients, better for hospitals, better for care homes and better for social care teams.
